Registering a New Passport in the Czech Republic – Application Form for Foreigners

Fill out or download the official form to report or register your new passport or national ID with the Czech Immigration Office (Ministry of the Interior).
You can either download the PDF or submit the form online. This registration is mandatory for foreigners living in the Czech Republic after any change to their passport or travel document.

 If you're a foreigner living in the Czech Republic and get a new passport, you must report and register it at the immigration office within 3 working days.

 If you lost your passport or ID, first report it at the local Czech police station. After you get a new passport, you must register it at the Czech immigration office (Ministry of Interior) within 3 days.

To register your new passport or national ID, you’ll need:

– Your new passport or ID – The official application form (you can download it or fill it out online on this page)

To report your new passport at the Czech immigration office, you need to download the application form (see above) and fill in the following details:

  • Jméno a příjmení – Your full name

  • Datum narození – Date of birth

  • Státní příslušnost – Nationality

  • Číslo pasu – New passport number

  • Telefon – Phone number

  • E-mail – Email address

In the section labeled “Druh změny”, check the box “Změna cestovního dokladu” (Change of travel document), and enter both your old and new passport or ID numbers

You might get a fine from the immigration office. It can also cause problems when travelling — for example, at the airport, border police may stop you if your new passport isn’t registered in the system.
