For the Permanent Residency application as an EU family member, you need:
Completed Application Form: You can download it above.
Two ID Photos: 3.5 cm x 4.4 cm with a white background.
Immigration Office Fee: Paid with value stamps (Kolky).
Accommodation Document: Rental contract or proof of accommodation.
Purpose of Stay Documents: Marriage certificate or proof of relationship for partners.
Valid Passport: Your current passport.
On the Permanent Residency Application Form for an EU family member, fill in:
Personal Information: Your personal details, marital status, and contact information.
Purpose of Stay: Indicate if applying for Permanent Residency after 5 years ("trvalý pobyt po 5 letech") or another relevant option.
Previous Stay in the Czech Republic: Details of any past residence longer than 3 months, if applicable.
Previous Residency: Your permanent address outside the Czech Republic.
Employment Details: Job information before and after moving to the Czech Republic, including your employer’s name, business address, and job position.
Family Members: Personal details and current addresses of your spouse, partner, or children.
Parents' Details: Personal details and current addresses of your parents. If deceased, write "zemřel/a" (deceased in Czech) in the address section.
For EU family members, proof of funds and Czech language exams are not required to obtain permanent residency in the Czech Republic.
